Adult Care Placement Services

Placement services assists aged or disabled adults who have no one available to assist them in finding substitute homes or residential health care facilities suitable to their needs when they are unable to remain in their current living situation.

Placement/Adult Foster Care Services include:

• Initial screening/assessment
• Help client/representative in securing needed medical information
• Assistance with completion of necessary financial application
• Help locate and secure placement in suitable setting and level of care

The individual needing services or someone acting on the individual's behalf can call the Adult Services line at 704-920-1400 to request assistance. Requests for assistance can also be made in person.
