Adult Day Care / Day Health

Adult Day Care/Health Services provides care and supervision to elderly or disabled adults, allowing caregivers to work or have respite and is an alternative to out of home placement. The program supports the elderly and disabled adults' desire to continue to live independently, reduce social isolation, loneliness, and stimulate interest in leisure activities.

In-Home Aide Services

In-Home Aide Services provides personal care and home management services to older adults and adults with disabilities who are unable to perform these tasks. Services are often provided to remedy or prevent abuse, neglect, or exploitation, and unnecessary out of home placement. These services are designed to provide assistance, not to replace the family in carrying out their responsibilities to individuals needing care and support.​​

Services For The Blind

The mission of the NC Services for the Blind (NCDSB) is to empower individuals who are blind or visually impaired to achieve their maximum potential through services provided by competent and caring professionals. The ILS program is designed to aid in the adjustment to vision loss with services including individual and family counseling, information and referral, support groups, recreational opportunities, instruction in home management, personal care and safe travel skills following vision loss.

Application process:

The Social Worker for the Blind, housed at the Department of Social Services, is in Cabarrus County Wednesday through Friday of each week. The application and assessment process is usually completed in the home setting. Anyone in the community may submit a referral, including physicians and human service agencies.

Special Assistance In-Home

Adults who are at risk of placement in an Adult Care Home may qualify for assistance to remain at home instead. The Special Assistance In-Home Program (SA/IH) can help qualified individuals who are assessed as having a safe home environment and demonstrated need for additional financial supports and services, to continue living independently. SA/IH can assist with essential expenses such as food, shelter, clothing and other daily necessities associated with a private living arrangement. A case manager works with participants to determine the need and purpose for SA/IH assistance. The case manager also monitors to assure the funds are used for the intended purposes and that the participant remains safely at home.

Eligibility criteria:

Must be at least 18 years old;Must require the same level of care as provided by an adult care facility (confirmed by MD) but express a desire to live at home or in another private living setting;Must have income below the federal poverty level and be eligible for Medicaid;If not already receiving Medicaid, must fill out an application and be determined eligible for both Medicaid and SA In-Home payment;Must demonstrate need for added financial support to remain safely at home;With services, can have health, safety and well-being maintained at home.

There are a limited number of SA/IH slots available in Cabarrus County and applicants may be placed on a Wait List. Once a slot becomes available, applicants will be advised to fill out an SA/IH Medicaid application and will then be referred to an SA/IH Case Manager for assessment in the home.
